Services may be provided in a person's home, the community (for example, shared living or assisted living), or institutional settings (for example, intermediate care facilities, hospitals, or nursing homes) to optimize their health and retain their independence.

People of all ages may need long term care. If you, a loved one, or a friend is an adult with a disability or an elder, you will find information about the DHS's publicly funded long term services and supports here.
