Healthy Transitions

Youth Works: Summer Jobs Program. This 6-week summer work program for young people (ages 14 to 24) residing in Woonsocket, North Smithfield, Cumberland, Lincoln, and Burrillville, also teaches resume writing, budgeting, and professionalism in the work place.

Healthy Transitions: Offers access to clinicians, a wraparound case manager, nurse, psychiatrist, substance abuse specialist, employment and training specialist who work as a team to assist participants with meeting personal goals for improving mental health and making successful transitions to adulthood.Anyone ages 16 - 25 living in the areas listed below may be screened for eligibility:Woonsocket, Burrillville, Cumberland, Lincoln and Smithfield.
