The Eleanor Slater Hospital provides specialized programs and services for people who are diagnosed with spinal cord injuries, traumatic brain injuries, dual diagnoses, neurological disorders, as well as chronic diseases.

The Zambarano Unit cares for the most complex patient population in the state. Referrals come from acute community hospitals, followed by nursing homes, independent and residential care facilities. Ventilators and dialysis are not available to Zambarano.

State operated, long term care hospital on an 80-acre site at Wallum Lake.

Medicare, Medicaid approved.

General Information



Intake Process:

When an application is completed, according to its instructions, it may be faxed to 401-462-6958, mailed to: Admissions Office, Regan Building, Eleanor Slater Hospital, P.O. Box 8269, Cranston, RI, 02920, or delivered in person. The additional documentation requested on the application form should be included.

When the application is received by the Admissions Office, it will be reviewed by the Admission Assessment Team. The team includes a physician, nurse, social worker, physical therapist, and psychologist.

If the Admission Assessment Team determines that the patient is a candidate for Eleanor Slater Hospital, the Admission Team will schedule an on-site visit, if necessary, to assess the potential candidate.

Once a candidate meets the criteria for admission to Eleanor Slater Hospital, placement will be made based on the specific type of medical services required: Acute, Long-Term, or Adult Psychiatric.
